TAMPA BAY AREA, Fla. — If you live in the Tampa Bay area your input is needed to better understand and address the health needs of your community.
The 2019 Community Health Needs Assessment (CHNA) surveys residents on nutrition, physical activity, access to care and mental health.
The Florida Department of Health in Hillsborough, Pasco, Pinellas and Polk counties and local area hospitals are partnering on the comprehensive survey.
Data collected from the CHNA survey will be used to develop a community health improvement plan, which will address the issues residents are most concerned about.
